In our lives, there are things that get in the way of doing the things we want. Sometimes there isn’t enough money. Sometimes you need a ride to the appointment, or the pharmacy. Other times, you just don’t understand what is happening.
These are called gaps in care. It means there is a disconnect or mismatch between what needs to happen and what is happening. It is impossible to take care of yourself when things like money, transportation, food, or understanding get in the way.
Just like any problem, there is a solution. It can be hard to find the solution, so I will offer up some strategies using transportation as an example.
Social workers are there to help you solve these problems. Your insurance, doctor’s office, charities, and hospitals either have one, or can help you locate one.
- Define the problem. Is the car working? Can you get in/out of the car? Do you need medical transportation?
- Communicate. Once you can explain your problem, talk to your doctor’s office about it. They may be able to do a phone or video visit. If you need more, ask for a social worker.
- Use your resources. A social worker is there to help close those gaps. They might find you medical transportation, a referral to a mechanic, or a caregiver to help you, depending on your needs.
- Be inventive. If you still can’t solve it, hit the internet. The hard part is knowing how to search.
- 211.org – You can also call 211 on the phone. It connects you to United Way and directs you to services that can help you.
- Benefits Checkup – The National Council of Aging put together a way to look for any benefits you are eligible to get.
- State or County website for services like aging, transportation, food banks, etc.
- Go to your disease website. Most have support services with help with the reality of managing the disease.
- Ask for help. You aren’t alone. Talk to your family, friends, or neighbors. They can be a valuable resource!
